Re: Grey Cloud Island Bridge Repairs
Background /Discussion
On September 8, 2010 the Minnesota Department of Transportation bridge unit performed an
inspection on the Grey Cloud Island Bridge as part of routine inspection of fracture critical
bridges. There was a critical finding in the inspection. Please see the enclosed L8159 Bridge
Critical Finding Documentation by the DOT. The critical finding of 25% loss of steel in an
interior and external channel and gusset plate has resulted in the need for the bridge to have <
load rating reduction to 22 ton for a single axle and 28 ton for a semi. These load ratings will
not allow Aggregate Industries' trucks to haul loaded over the bridge. This will impact the
export of material from their facility. It is critical for this repair to occur quickly.
SRF was contracted to prepare plans for the repair, since they had completed the load rating
analysis in 2009 for the bridge. They completed the plans and had DOT approval on
September 22, 2010. The plans were packaged and distributed to four companies to provide
quotes for the work. The engineers estimate for the work is $20,000 and has a completion
date of December 6, 2010. This is an urgent non - budget item.
The following are the results of the quotes received:

It is recommended the Council adopt a resolution awarding the quote for the Grey Cloud Island
Bridge repairs to High Five Erectors in the value of $16,100.

RESOLUTION AWARDING QUOTE FOR GREY CLOUD ISLAND BRIDGE NO.
L8159 TO HIGH FIVE ERECTORS, INC. IN THE AMOUNT OF $16,100.00
WHEREAS, Plans and specifications were completed according to
city standards, and
WHEREAS, quotes were requested to provide the necessary work;

WHEREAS, two firms submitted quotes; and
WHEREAS, it appears that High Five Erectors, Inc. is the lowest
responsible quoter; and
WHEREAS, it is the recommendation of the City Engineer that the
contract be awarded to High Five Erectors, Inc.
NOW THEREFORE BE IT RESOLVED, by the City Council of the City
of Cottage Grove, County of Washington, State of Minnesota, that the quote for
the Grey Cloud Island Bridge No. L8159 to High Five Erectors, Inc. in the amount
of $16,100.00.

Bridge L8159 Critical Finding Documentation
September 8, 2010 Inspection
Location: Grey Cloud Trail over Channel
Owner: City of Cottage Grove, MN in Washington County
Team Lead Inspectors Present: Jennifer Zink and Ken Rand
Assistant Inspectors & Others: Ryan Rohne
During the fracture critical and routine inspection on September 8, 2010 at Bridge L8159, the following
finding initiated a critical finding per Mn /DOT Technical Memorandum 08- 02 -B -02:
Truss Span — LO -Ul Diagonal Member Web Compression Member — (2) C8 x 11.5 x 1 /4 "PL
• East Truss (Photos 1 -2): Web through corrosion on interior channel near interface with gusset LO
East. Photo taken during the 2009 routine inspection shows no through corrosion at that time.
• West Truss (Photo 3): The web on both the interior and exterior channels near the interface with
gusset LO West have 25% loss overall. There is also 12% loss at the bottom chord exterior angels
at the LOW gusset plate and 25% loss along the horizontal shear zone of the exterior gusset plate
for a length of 6 inches (Photos 4 -5). The gusset plate is 3/8" thick. 33% loss was accounted for
in the gusset horizontal shear area however in the 2009 SRF load rating.
LO' -Ul' Member does exhibit developing corrosion in same area, but not to this extent yet (Photos 6 -7).
Tom Styrbicki - Mn /DOT Bridge Inspection Program Manager, Yihong Gao — Mn /DOT Load Ratings, and
Todd Niemann — Mn /DOT Bridge Inspection Engineer were contacted by Jennifer Zink onsite with the
finding via e-mail with photo. It was determined that the finding be forwarded to the City Engineer,
the consultant routine inspector, SRF Consultants — who performed the last load rating in 2009, and
Dave Conkel — Mn /DOT Bridge State Aid Engineer. SRF completed a preliminary load rating on
September 9, 2010:
OPERATING RATING = 0.79 (HS15.8)
SINGLE UNIT TRUCK = 0.95 (22.8T)
SEMI (3S2 -40) = 0.79 (31.6T)
DBL (3S3) = 0.74 (29.6T)
Indicated posting: 22T/28T/28T
As a result of the load rating analysis, the City retained SRF to prepare repair plans. The City intends
to temporarily post the bridge until repairs are made.
The Mn /DOT Bridge Office updated the Pontis inventory and inspection report to reflect the following:
NBI Superstructure: 2
Critical Finding Smart Flag #964: 2
Fracture Critical Smart Flag #966: 3
The critical finding will resolve to condition state 1 once verification of the load posting signs are in
place. The NBI Superstructure rating will also be upgraded to 4 and the Fracture Critical Smart Flag to
2 — please reference the Mn /DOT Bridge Inspection Manual Version 1.8 for details on condition states.
Further condition state changes are anticipated once repairs are made.
