Proposal
Verizon Wireless has applied for conditional use permits to allow installation of nine antennas on
the existing lattice tower and a 50- kilowatt natural gas generator at 7516 80th Street. All nine of
the panel antennas will be mounted at the 62 -foot elevation of the 120 -foot tower. Three of the
nine panel antennas are approximately six inches wide and eight feet in length. The other six
panel antennas are approximately six inches in width and three and one -half feet in length.
The proposed natural gas generator will be located ten feet southeast of the existing lattice
tower. The generator will be placed on a concrete pad that is four feet by eight feet.
The installation of these antennas will improve coverage and capacity to support current and
future wireless communication customer needs. This is the site of the Business Enterprise
Center (BEC) building located at 7516 80th Street South.

Planning Considerations
Ordinance Criteria
The City's Electromagnetic Communication Facilities ordinance requires a conditional use per-
mit for antennas. Approval is subject to the provisions of the zoning ordinance and any condi-
tions required by the City. Verizon's application complies with the ordinance regulations.

Planning Considerations
Equipment Storage and Generator
The wireless communication equipment will be placed inside the attached garage /storage space
for the BEC facility.
The applicant is proposing to install a natural gas generator approximately ten feet southeast of
the existing lattice tower. The proposed natural gas generator will power Verizon Wireless'
equipment. This area of the property slopes away from the garage exterior wall and is
landscaped with concrete edging. To construct a concrete slab for the generator and the two air
conditioner units, the existing landscaping must be removed and re- graded. As an alternate lo-
cation, the City will propose to the applicant the area on the north side of the garage facility near
the northwest corner of the garage facility. If the proposed natural gas generator will remain on
the east side of the garage structure, the generator and air conditioner units must be screened
from view. A privacy fence a minimum of six feet in height is an option with landscape plantings
and mulch along the fenced area. All yard areas and underground irrigation systems disturbed
by construction must be replaced with similar materials.
The existing diesel generator located on the north side of the garage /storage area will remain on
the property. This generator powers the building in the event of power failure.
Antennas
Nine panel designed antennas will be mounted at the 62 foot elevation on the 120 foot lattice
tower. Three of the nine panel antennas will be approximately six inches wide and eight feet in
height. The other six panel antennas will be approximately six inches wide and three and one -
half feet long. An aerial photo and elevation of the lattice tower and the proposed location of the
Verizon Wireless antennas are shown below.

The applicant has explained that the installation of these new antennas will improve the Verizon
Wireless coverage area.
Public Hearing Notices
Public hearing notices were mailed to 73 property owners who are within 500 feet of the pro-
posed site. These notices were mailed on August 14, 2013.
Recommendation
That the Planning Commission recommend approval of Verizon Wireless' conditional use permit
application to install nine panel antennas and a natural gas generator as shown their plans
dated June 6, 2013; subject to the conditions listed below:
1. All applicable permits (i.e., building, electrical, mechanical) and a commercial plan review
packet must be completed, submitted, and approved by the Building Official, Fire Marshal,
and Public Works Director prior to the commencement of any construction activities.
2. The applicant must schedule a pre- construction meeting with the Public Works Director
before work can begin.
3. Construction or installation of equipment on the subject property may only commence once
the applicant/permittee has entered into an agreement with the City for leasing city -owned
land or buildings, access, antenna installation on the lattice tower, and all other details rec-
ommended by the City Attorney. The lease agreement must be signed by the applicant

before the agreement and the Planning Commission's recommendation for the conditional
use permit are placed on the City Council agenda for final action.
4. Advertising on the antennas is prohibited.
5. Outdoor storage is prohibited.
6. Color and sample materials for all cables, antennas, and mounting brackets /braces must be
submitted to the Public Works Director for final approval prior to issuance of the building
permit. City specified manufactured colored coaxial cable will be required (painted coaxial
cable will not be accepted).
7. No significant change in the use or design of the antennas may be made without the Public
Works Director's consent. Minor modifications to service or use may be considered if such
changes will not cause the antennas to violate the performance standards set forth in the
City Codes or the conditions of this conditional use permit.
8. Utilities must be installed underground as approved by the Public Works Director.
9. The applicant must submit detailed construction plans and specifications of the purposed
antenna facilities, related equipment, and screening /landscaping improvements to the De-
partment of Public Works for review. All plans and specifications including the location and
attachment of the proposed antenna equipment will require final approval from the Public
Works Director prior to the issuance of a building permit.
10. If the proposed natural gas generator remains on the property, the generator and air con-
ditioner units must be screened from view. A privacy fence a minimum of six feet in
height is an option with landscape plantings and mulch along the fenced area. All yard
areas and underground irrigation systems disturbed by the proposed improvements must
be replaced with similar materials.
11. Any modification to the existing grades must not negatively impact existing drainage
swales or the building's exterior walls and /or foundation.

The purpose of this letter is to provide supporting information to the Conditional Use Permit Application
that has been submitted by Verizon Wireless for the address listed above. Verizon Wireless respectfully
requests the City of Cottage Grove to review this proposal and grant the Conditional Use Permit to
install antennas and equipment cabinets at this location.
The tower is currently located behind the garage at this site. Verizon would propose to install their
antennas on the existing tower and locate their equipment cabinets in the garage. The NE corner of the
inside of the garage would become an equipment room to house the cabinets for Verizon. A generator
is proposed outside the garage on the NE corner near the tower.
The property is currently zoned as B1 Limited Business and is planned to be zoned as Commercial in the
City's Comprehensive Plan.
The proposed installation of the above mentioned facility is planned for the fall of 2013. There are
several capacity issues in this area which has caused dropped calls, slow connection or no connection to
the Verizon network.
