COUNCIL ACTION REQUEST
Pass a motion adopting a resolution accepting a grant from the Shakopee Mdewakanton Sioux
Community for two Philip FRx AED monitors with AED defibrillation pads with an estimated
value of $1,500.00 each which will be dispersed to the police and fire departments to be used
in cardiac emergencies.

Subject: Shakopee Mdewakanton Sioux Community AED Grant Award
INTRODUCTION
The Cottage Grove Fire Department applied for a grant from Shakopee Mdewakanton
Sioux Community early March, 2014. The grant was for their Mdewakanton Life
Program and the request was for two AEDs. I have received a letter indicating we have
been awarded two Philips FRx AEDs, pads and cases. The Philips AEDs will be
dispersed to the police and fire department to be used in cardiac emergencies.
BACKGROUND
The purpose of the Mdewakanton LIFE Program is to create greater awareness,
availability, and knowledge of cardiopulmonary resuscitation (CPR) and automatic
external defibrillators (AED's), as well as, a faster first response to a cardiac incidence
not only within our community but within our neighboring communities as well. To
achieve these goals, the Shakopee Mdewakanton Sioux Comrnunity is reaching out
through the Mdewakanton Fire Department to train first responders and place AED units
throughout our area.
ACTION REQUESTED
Request Council adopt a resolution to accept two cased Philip FRx AED monitors with
AED defibrillation pads from Mdewakanton Emergency Services and Laura LaFavor.

Congratulations, the Shakopee Mdewalcanton Sioux Community Business Council has approved for your
organization to receive two Heai�tStaT�t Fl� automatic electronic defibrillators. I�i reading your requests, it
appears that AED wall-mount cabinets will not be needed. Is that correct?
The order will ship to us next week. Please let me know what day and time works best for you to coine to
our station inid-next weelc or the following week to receive the donation. I will coordinate with
Community Meinbers Nate Crooks and Sheldon Biyant who also work here at Mdewakanton Public
Safety to also be here for the donation and photo.

RESOLUTION NO. 14 -
RESOLUTION ACCEPTING GRANT AWARD TO
THE DEPARTMENT OF PUBLIC SAFETY
FIRE DIVISION
WHEREAS, the Cottage Grove Department of Public Safety, Fire Division has
applied for a grant through the Shakopee Mdewakanton Sioux Community early March,
2014 for a grant request for two Automatic Electronic Defibrillators (AED) to be used by our
Police and Fire Departments in cardiac emergencies, and
WHEREAS, the City of Cottage Grove, on behalf of the Department of Public Safety,
accepts the awarded Philip FRx AED monitors with AED defibrillation pads and cases at an
estimated cost of $1,500.00 each.
NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of Cottage
Grove, County of Washington, State of Minnesota, that said Council hereby accept, on
behalf of the Department of Public Safety Fire Division, the total grant award of two Philip
FRx AED monitors with AED defibrillation pads and cases at an estimated cost of $1,500.00
each ($3,000.00 total) .
