Also included are the IC 134 forms and Lien Wavers as submitted by the Contractor. The contractor for this project is Max Steininger, Inc. REVIEW OF PROJECT HISTORY The contractor began this project in September of 2014 and the project was substantially completed in October of 2014. The contractor returned in the spring and summer of 2015 to complete the remaining punch list work. CONSTRUCTION COST OVERVIEW As indicated on the final request for payment, the original contract amount was$417,845.98. The contract was amended to include Change Order No. 1 for$5,820.94. This provided compensation to the Contractor for additional signage needed for a detour, speed reduction and the signal, as well as hand hole extensions, additional excavation of the trail to match grade of the pedestrian ramp on the northeast quadrant, and hauling poor soils off site and replacing it with sand. The revised contract amount is$423,666.92. The final construction cost as shown is $421,585.44 which is $2,081.48 less than the revised contract amount. The reduced construction cost was primarily due to underruns in item such as excavation. DESIGNING FOR A BETTER TOMORROW Bolton&Menk is an equal opportunity employer . / RECOMMENDATION Max Steiniger, Inc., in accordance with the contract plans and specifications has satisfactorily completed this work. Therefore, it is recommended that final payment of$29,922.97 is made and the City of Cottage Grove accepts the project. After acceptance, please distribute all requests for payments as appropriate.
