Staff Recommendation:Appoint Rachel Smoot to fill the vacant Economic Development Authority position for a
term ending on December 6, 2023.
G Massage Business and Therapist Licenses - Holly Thomas
Staff Recommendation:Authorize issuance of a massage business and massage therapist license to Holly Thomas
to operate as a home occupation at her residence at 9444 67th Street South.
H Massage Therapist License - Larissa Schaar
Staff Recommendation:Authorize issuance of a massage therapist license to Larissa Schaar to work at The Healing
Place, 8617 West Point Douglas Road South.
I 2019 Joint Crack Sealing Project -Quote Award
Staff Recommendation:Adopt Resolution 2019-048 awarding the 2019 Joint Crack Sealing Project to the lowest
accepted quote supplied by Fahrner Asphalt Sealers, LLC in the amount of$64,264.56.

Staff Recommendation:1. Receive the draftADA Transition Plan. 2.Approve posting the draftADA Transition Plan
for public comment.
K Verizon Wireless —Conditional Use Permit
Staff Recommendation:Adopt Resolution 2019-049 approving the conditional use permit for the proposed
installation of six antennas by Verizon Wireless on top of the Ciry's Meadow Grass water reservoir.
L Parkview Pointe Utility, Street, and Lighting Improvements —Approve Plans & Specifications
and Establish Bid Date
Staff Recommendation:Adopt Resolution 2019-050 approving the plans and specifications and establishing a May
9, 2019, bid date for the Parkview Pointe Utiliry, Street, and Lighting Improvements.
M Rental License Approvals
Staff Recommendation:Approve the issuance of rentallicenses to the properties in the attached table.
N Danner Grading Permit for Apache Site - Interim Conditional Use Permit
Staff Recommendation:Adopt Resolution 2019-052 approving the Interim Conditional Use Permit to allow the
continued grading of the 70.23-acreApache site as proposed on the grading plan dated February 15,2019, as
prepared by Pioneer Engineering.
O Summers Landing 3rd Addition Utility, Street, and Lighting Improvements—Approve Plans &
Specifications and Establish Bid Date
Staff Recommendation:Adopt Resolution 2019-051 approving the plans and specifications and establishing a May
9,2019, bid date for the Summers Landing 3rd Addition Utiliry,Street, and Lighting Improvements.
P Therapeutic Massage Ordinance Amendment
Staff Recommendation:Adopt Ordinance No. 1003 amending City Code Title 3, Chapter 8, regarding massage
therapist licensing.
Q Sale of Fire Engine 2
Staff Recommendation:Authorize the sale of Engine 2 (1990 PeterbiltJ via a Fire Apparatus Auction Site,posting an
ad vertisement for fire departments, or through the public auction site CrankApe.com.
R Fa�ade Improvement Program Award
Staff Recommendation:Authorize staff to submit an award letter and contract for the Oasis Chiropractic and
Wellness Building project for$25,000 as part of the Fasade Improvement Grant Program.
S 7470 East Point Douglas Road — Request to Submit a Letter of Intent (LOI)
Staff Recommendation:Authorize the EDA Executive Director/Ciry Administrator to submit a Letter of Intent(LOIJ
to Re/Max Results Commercial Group for the negotiation and purchase of 7470 East Point Douglas Road and begin
additional environmental investigation work.

A Special Event Permit- Strawberry Fest
Staff Recommendation:Authorize issuance of a special event permit to Cottage Grove Area Strawberry Festival,
Inc.for events associate with Strawberry Fest contingent upon compliance with conditions outlined in Public Safety
Director Koerner's memo dated 4/11/2019.
