C6ange Order's # 29 and 39 Listul Industries Change Order #29 deals with (i) A handrail not shown on plans, however it was required by code. (2) Additional angle 'uon installed at the roof on the new office area not detailed on the plans, (3) A credit for steel lintels previously covered under Keilington's contract, and (4) The addition of a stee) beam and columns in the Men's Locker room. This was necessary due to the fact that a wall previously thought to be a non-bearing wall was indeed a bearing wali. This was an unforeseen condition, not shown on the record drawings. Change Order #39 deals with (i) A backcharge to Kellington Construction for work perFormed by Listul. This cost is added Yo ListuPs contract and deducted from Kellington's in Change Order # 35. This is a no cost item to the City. This work was not in original contract. (2) Deduct for work performed by Listul Industries. (3) Footing elevation assistance that was provided as per the contract on a per hour basis. (4) Sidewalk at the front entry to the new office. (5) Field Change Orders #8535 ( added concrete aprons between garage doors) #8536 (cauiking of all control joints not shown on plans) #9837 ( additional block and footing demolition not shown on p2ans) (additional control joints in masonry not on plans) (6) Field Change Orders # 9242 ( additionai slab stoop poured at the wood shop) #9253 ( removal of the windows at the North wall of the Locker room to facilitate installation oF separation wall. Demolition of windows was not on plans.) #9255 ( removal and salvaging of hardware from (2) shower stalls that were not installed due to handicap restrictions.} #9842 ( Masonry infill at windows in Locker room area not on plans.) (7) Field Change Order #2451 Bush down and install slope in shower floors. (existing) (7) Cost associated with the addition of posts and beams in the locker room area. This was the bearin� wall previously talked about. In the Listul Change Order # 29. (8) Furnish and install aluminum threshold and sweeps at the double doors to the carpenter shop as per pwner request. (9) Additional drywall in the Training, Locker Room and Mezzanine area as per Owner request. (10) Cost associated with new bathroom at hallway. ( demolition, block work, door, hardware, toilet hardware, mirror. (l I) Cost associated with removal and replacement of door 124B. This shouid have been specified as a fire fated door. (12) Cost to remove and reinstall the concrete floor in the Training room. (2) Abandon well pump and controls. (3) Disconnect and reinstallation of furnaces, damper motors, eathaust fans and unit heater in mezzanine area due to the installation of additional baz joists. Plans for mechanical and electrical did not address this area. (4) Connect and disconnect temporary heaters. (5) Relocation of PVC stub up's in the new office area due to architectural measurement discrepancies and conflict with window measurements. (6) Relocation of 200 amp service at pole shed as per Owner request. (7) Additional low voltage piping at mechanics office as per Owner request. (8) Installation of Owner supplied light fixtures in sign shop as per Owner request. (9) Reinstallation of 4 fi�ctures in Mechanical room due to addition of lay in ceiling not on plan. (10) Disconnect and reconnect mechanical units on e�cisting roof for re-roofing. (11) Additian of salvaged Ix4 fluorescent fixtures at training room. (12) Fumish 2 additional wall mount lights at front entry to new office. Change Order # 41 deals with the following issues; (1) A credit for trenching done by the City for Phasor. (2) Wiring required at the Vehicle lift. This was previously on the documents as an alternate. (3) Wiring required at gas pumps as per Owners request. (4) Addition of outiets and data access boxes in Foreman's room as per Owner request. (5) Connection of exhaust fans for welding hood and vehicle eachaust system in Mechanics bay. Not shown on electrical plans. (6) Addition of EM ballast (aight light} at Lunch room 121 as per F'ue Mazshall instruction. (7) Re- pull make up sir units # 1 and 2 feeds with neutral wires. Required but not on plans. (8) Repair of damaged undergound canduit and wiring at Locker Room area where slab was demolished. (9} Addition of 3-way switching at Mezzanine and stair as per Owner request. (10) Addition of CATV receptaoles in Training room as per Owner request. (11) Additional receptacle at retrofit water cooler. (12) Additional light sensor in retrofit toilet as per Owner request. (13} Fumish and instaii added wire mold for computer ternunal in Mechanics Office.
